Forged Mens Health LLC (“Forged Mens Health,” “we,” “us,” or “our”) operates forgedmenshealth.com and provides men’s health coaching, education, community, and related services. This Privacy Policy explains what personal information we collect, how we collect and use it, who we share it with, and the choices you have regarding your information.

By using our website, signing up for our newsletter, booking a consultation, joining our community, or enrolling in any program, you agree to the practices described here.

Our services are intended for adults 18 and older. See the Children’s Privacy section below.

1. INFORMATION WE COLLECT

We collect the following categories of information:

Information you provide directly to us:

  • Identifying information, including your name, email address, phone number, and mailing or billing address.

  • Payment information submitted when purchasing a program or service. Card details are collected and processed by Squarespace Payments and are not stored directly by us.

  • Health and intake information you voluntarily provide through our onboarding form and through PracticeBetter, our practice management platform. This may include medical history, current symptoms, medications and supplements, lab results (such as hormone panels, GI-MAP, and genetic testing you choose to share), lifestyle and nutrition details, and your goals.

  • Communications you send to us, including support messages, survey responses, and community posts.

  • Account and profile information you create within ClientClub (our paid community) and PracticeBetter.

Information we collect automatically:

  • Usage data, including the pages you visit, links you click, time spent on the site, and how you arrived.

  • Device and technical data, including your browser type, operating system, IP address, and approximate location.

  • Cookies and similar tracking technologies placed by Google Analytics and the Meta Pixel. See the Cookies and Tracking section below.

2. HOW WE COLLECT INFORMATION

We collect information through:

  • Web forms on our site, including our onboarding intake form.

  • Newsletter signup managed through GoHighLevel (GHL).

  • Consultation and appointment bookings made through PracticeBetter.

  • Purchases and payment transactions processed through Squarespace Payments.

  • Account creation and activity within ClientClub.

  • Analytics and advertising technologies, including Google Analytics and the Meta Pixel, which use cookies and similar technologies to collect usage and device data.

3. HOW WE USE YOUR INFORMATION

We use the information we collect to:

  • Deliver, operate, and support the programs, coaching, consultations, community access, and educational content you have requested.

  • Communicate with you, including sending appointment reminders, service updates, and responses to your inquiries.

  • Send newsletter and marketing communications where you have signed up or where otherwise permitted by law. You can opt out at any time using the unsubscribe link in any email or by replying STOP to any text message.

  • Process payments and manage your account, billing, and enrollment.

  • Understand how our site and services are used so we can maintain and improve them.

  • Measure and improve our advertising through Google Analytics and the Meta Pixel.

  • Comply with our legal, regulatory, and professional obligations, and protect the rights, property, and safety of our clients, our business, and others.

4. THIRD-PARTY SERVICE PROVIDERS

We work with third-party platforms and partners to operate our business. These third parties receive only the information necessary to perform their functions and each maintains its own privacy policy. We encourage you to review those policies. Our current third-party relationships include:

  • PracticeBetter — practice management, intake forms, secure client portal, and consultation scheduling.

  • ClientClub — hosting and management of our paid community.

  • GoHighLevel (GHL) — newsletter, email and SMS communications, and marketing automation.

  • Squarespace — website hosting and platform.

  • Squarespace Payments — payment processing.

  • Fullscript — supplement dispensary platform through which supplement recommendations may be fulfilled. See our Terms and Conditions for disclosure of our commercial relationship with Fullscript.

  • Meta (Facebook and Instagram) — advertising and the Meta Pixel.

  • Google — Google Analytics and related measurement services.

We do not sell your personal information for money. Please see the California Privacy Rights section for how certain advertising and analytics activity may be treated under California law.

5. SMS AND TEXT MESSAGING

If you provide your phone number and consent to receive text messages, we may send you appointment reminders, program updates, and marketing communications via SMS through GoHighLevel. Standard message and data rates may apply. You can opt out of text messages at any time by replying STOP to any message we send. This opt-out does not affect your email communications.

6. HEALTH INFORMATION

Forged Mens Health provides health coaching and education to a national audience. We do not provide diagnosis, treatment, or prescribing. We treat health-related information as confidential and limit access to those who need it to provide your services.

Note regarding HIPAA:

Based on the cash-pay, direct-to-consumer structure of our coaching and education services, Forged Mens Health does not believe it operates as a “covered entity” under the Health Insurance Portability and Accountability Act (HIPAA) in connection with those services, because we do not bill health insurance or conduct the standard electronic transactions that trigger HIPAA coverage. Even so, we treat your health information as confidential and protect it using reasonable administrative, technical, and physical safeguards.

Information you share through PracticeBetter is handled in accordance with that platform’s privacy and security practices. Information you post in ClientClub is not confidential and may be visible to other community members. Use discretion when sharing personal health details in community spaces.

7. COOKIES AND TRACKING

Our website uses cookies and similar tracking technologies to operate the site, remember your preferences, analyze traffic, and measure our marketing. The technologies we currently use include:

  • Google Analytics — to understand how visitors use our site and to improve performance and content.

  • Meta Pixel — to measure the effectiveness of our advertising and to deliver relevant ads on Facebook and Instagram.

  • Google Ads conversion tag — to measure which of our ads lead to bookings and to show relevant ads to people who have already visited our site.

  • LinkedIn Insight Tag — to measure the performance of our LinkedIn advertising and to show relevant ads to people who have already visited our site.

When you first visit our site, a cookie banner will allow you to accept or decline non-essential cookies. You can also manage cookies at any time through your browser settings; disabling certain cookies may affect how parts of the site function.

For interest-based advertising choices, visit your Google ad settings and Meta ad preferences. You can also use opt-out tools available through the Digital Advertising Alliance at optout.aboutads.info.

Do Not Track: Our website does not currently respond to browser Do Not Track signals. We will update this policy if our practices change.

8. DATA RETENTION

We retain your personal information for as long as needed to provide our services, maintain your account, comply with our legal and professional recordkeeping obligations, resolve disputes, and enforce our agreements. Health-related records are retained in accordance with applicable professional and legal requirements. When information is no longer needed, we take reasonable steps to delete or de-identify it.

In the event of a data breach affecting your personal information, we will notify you as required by applicable state and federal law.

9. DATA SECURITY

We use reasonable administrative, technical, and physical safeguards to protect your information against unauthorized access, use, alteration, and disclosure. We also rely on the security measures of the established platforms we use, including PracticeBetter and Squarespace Payments. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

10. YOUR CALIFORNIA PRIVACY RIGHTS (CCPA/CPRA)

If you are a California resident, the California Consumer Privacy Act as amended by the California Privacy Rights Act (CCPA/CPRA) gives you certain rights regarding your personal information. Subject to the law’s exceptions, these include:

  • The right to know what personal information we collect, use, disclose, and share, including the categories and specific pieces of information.

  • The right to access a copy of the personal information we have collected about you.

  • The right to delete personal information we have collected from you.

  • The right to correct inaccurate personal information we maintain about you.

  • The right to opt out of the “sale” or “sharing” of personal information. We do not sell your personal information for money. However, certain analytics and advertising activity — such as the use of the Meta Pixel and Google Analytics — may be considered “sharing” or a “sale” under California law. You can opt out by using our cookie banner or adjusting your browser settings. Our cookie banner includes a “Do Not Sell or Share My Personal Information” option as required by California law.

  • The right to limit the use of sensitive personal information to what is necessary to provide our services.

  • The right to non-discrimination for exercising any of your privacy rights.

Our website and services are operated in the United States. Some of our third-party service providers, including Meta and Google, may process data in the United States or other countries. By using our services, you acknowledge that your information may be transferred to and processed in countries outside your own, which may have different data protection laws.

12. CHILDREN’S PRIVACY

Our services are intended only for adults 18 years of age and older. We do not knowingly collect personal information from anyone under 18. If you believe a minor has provided us with personal information, contact us at chad@mail.forgedmenshealth.comand we will take steps to delete it.

13. GOVERNING LAW

This Privacy Policy is governed by the laws of the State of Arizona, without regard to its conflict-of-law provisions. Any disputes arising under this policy will be resolved in the state or federal courts located in Arizona, and you consent to personal jurisdiction in those courts.

14. CHANGES TO THIS POLICY

We may update this Privacy Policy from time to time. When we do, we will revise the “Last Updated” date at the top of the page. Material changes will be communicated through our website or by other appropriate means. Your continued use of our services after an update constitutes acceptance of the revised policy.
